A Canadian one-of-a-kind home telehealth monitoring program developed by the University of Ottawa Heart Institute (UOHI) has cut hospital readmission by 54% for heart failure patients. "Safety-critical industries such as the airline sector have successfully employed checklist systems to ensure safety and quality of services. The University of Ottawa Heart Institute has adopted the same approach but with a checklist geared to heart attack patients. These patients are sent home with a complete discharge checklist that includes appropriate pre scriptions for medications such as ACE inhibitors and beta blockers. It also includes referrals and education for smoking cessation, nutrition and exercise programs. Items on the checklist include Aspirin and other necessary drugs and procedures." Read more...
